Special Supplemental Benefits for the Chronically Ill (SSBCI)

The plan offers Special Supplemental Benefits for the Chronically Ill, also called SSBCI. These are extra benefits for members who have certain long-term health conditions and meet plan rules. These benefits can help with healthy food and living costs, such as rent and utilities.

Who qualifies

To qualify, you must have one of these health conditions:

  • Cardiovascular disorders
  • Chronic and disabling mental health conditions.
  • Chronic gastrointestinal disease (limited to end-stage liver disease)
  • Congestive heart failure
  • Connective tissue disease
  • Chronic lung disorder (limited to chronic obstructive pulmonary disorder)
  • Dementia
  • Diabetes mellitus
  • Overweight, obesity, and metabolic syndrome
  • Stroke

You must also meet one of these rules:

  • You had one or more hospital stays related to your condition in the past 12 months. This includes behavioral health stays.
  • You had two or more doctor or outpatient visits related to your condition in the past 12 months.
  • A need for home health visits related to your condition.
  • You need certain medical equipment at home or outside the hospital, such as a wheelchair, BiPAP machine, or special mattress.
  • You had one or more urgent care or emergency room visits related to your condition in the past 12 months.

Find out if you’re eligible

You may already qualify for SSBCI based on your medical history. If you think you are eligible, ask your provider to confirm and complete the online provider attestation process. You do not need to submit anything. For more information, please click here. 

How it works

If you qualify, you will get a $58 monthly allowance on a debit card from the plan. You can use the card to help pay for everyday living costs.

Note: Any money you do not use expires at the end of each month or when you leave the plan.

Use your plan debit card at participating retailers when you buy eligible items or services. The card is linked to your benefits and checks eligibility at checkout.

Eligible purchases

You can use your card only for benefits and over-the-counter items covered by your plan. Not all items in a store are covered. For a full list of covered items and benefits, visit www.andmorehealth.com.

Examples: Covered items may include healthy foods, over-the-counter medicines, or other approved plan items. Covered services may include help with general supports for living (rent, mortgage,utilities and internet), and healthy foods, if you qualify.

If your card does not work

If your card does not work at checkout, you may pay for the covered item or service yourself. This is called paying out of pocket. After you pay, you can ask to be paid back. This is called reimbursement. To ask for reimbursement, save your receipt and make sure it shows the item or service name, the date you paid, and the cost. 

You will also need to include your full name and mailing address. Visit www.andmorehealth.com to submit your request online. 

  1. Pay for the covered item or service.
  2. Keep your receipt or proof of payment.
  3. Check that the receipt shows what you bought, when you paid, and how much you paid.
  4. Submit your request online at www.andmorehealth.com/#. You will be required to upload an image of your receipt and will select whether you would like to be paid back directly to a bank account, debit card, or via check. 
  5. Wait for your request to be reviewed.

Example: If your card does not work when you buy a covered item, you can pay for it yourself, keep the receipt, and ask to be paid back. For example, if you buy approved over-the-counter medicine, your receipt should show the medicine name, the date, and the price.

Using your card for rent, mortgage, or utility bills

You can use your allowance to help pay rent, mortgage, and utility bills. You must pay the bill first. Then you can ask to be paid back. For example, before you start, have your bill, lease or mortgage statement and proof you paid. Proof may include a receipt, bank statement, canceled check, or payment confirmation. Visit www.andmorehealth.com and follow these steps:

Example: If you pay your electric bill, you may upload a copy of the bill and proof that you paid it, such as a receipt or bank statement.

  • Scroll to the bottom of the page and click Reimbursements.
  • On the Reimbursements page, you can review past requests or start a new one.
  • To start a new request, choose rent payment or utility bill.
  • Follow the steps on the screen. Choose your payment method, enter the requested information, upload your lease if needed, and upload proof that you paid.

After you submit your request, it will be reviewed. If more information is needed, you may be contacted. Keep copies of your receipts, bills, and proof of payment until your request is complete.

These benefits are part of a special program for members with long-term health conditions. Not all members qualify. Only members who qualify for SSBCI may use their allowance for general supports for living (rent, mortgage, utilities and internet),  and healthy foods To qualify, members must have at least one approved chronic health condition, such as a heart condition, congestive heart failure, chronic lung disease, diabetes, or a chronic and disabling mental health condition.
