• The Notice of Availability (NOA) (PDF) under Section 1557 of the Affordable Care Act (ACA) informs individuals that our health plan will provide free language assistance and auxiliary aids or services for those with Limited English Proficiency (LEP) or disabilities.

How to enroll — Enrollment begins October 15

There are five ways for Medicare beneficiaries to enroll in AmeriHealth Caritas VIP Care.
Pick the way you prefer:

1. Home visit

Call and schedule an appointment for a home visit with one of our representatives toll-free at 1-800-858-1487 (TTY 711) for Miami-Dade and 1-800-898-3018 (TTY 711) for other counties, Monday through Friday, 8 a.m. – 8 p.m., from April 1 to September 30; or seven days a week, 8 a.m. – 8 p.m., from October 1 to March 31.

2. By phone

Call and enroll over the phone with one of our representatives toll-free at 1-800-858-1487 (TTY 711) for Miami-Dade and 1-800-898-3018 (TTY 711) for other counties, Monday through Friday, 8 a.m. – 8 p.m., from April 1 to September 30; or seven days a week, 8 a.m. – 8 p.m., from October 1 to March 31.

3. Online

• Fill out and submit the online enrollment form.

4. By mail

• Download and print the enrollment form/attestation (PDF), the Creole enrollment form/attestation (PDF) or the Spanish enrollment form/attestation (PDF). Complete the form in full, sign, date, and mail them to:

5. Centers for Medicare & Medicaid Services (CMS) online enrollment center

• Medicare beneficiaries may also enroll in AmeriHealth Caritas VIP Care through the CMS Medicare Online Enrollment Center located at Medicare.gov.
